Guys balls don't actually turn blue. I would say its more of a term rather than a description. It happens if a guy has been really turned on for a while and gotten close to cumming, but hasn't actually cum. Then as he looses his erection he gets a kind of painful aching in his balls that takes a few hours (sometimes more) to go away.
