Blue balls is a not quite scientific term used to describe the sensation that a guy gets if he's been aroused (boner) for a period of time, without achieving orgasm. Generally, if he's hard for a while, and doesn't get to orgasm, the blood which caused his erection can sometimes have a difficult time exiting the penis.
Technically, it's a real thing, in that guys can actually get a sort of cramp/painful sensation in the groin, when that happens. It's easily solved, though. Regardless of what excuse a guy comes up with, masturbation will take care of it, just fine.
