To echo what @Orion1084 said, the location, type, and frequency of pain are all important. If it happened once and has never happened again, it may have just been something muscular in that particular moment. If it happens every time he ejaculates, that is definitely more serious. If it is a burning pain in his shaft, that could indicate some sort of infection in his urinary tract that is causing inflammation in his urethra. If it is in his balls, that could indicate that something in there is either growing or maladjusted. If it feels deeper there could be an issue with his prostate. Either way, if it happens multiple times, he should see a doctor. It may be embarrassing, but you really don't want to mess around when it comes to pain in that part of the body.

Having a bladder infection or a urinary tract infection (UTI) is not something to ignore. It can lead to a KIDNEY INFECTION!

If he is pissing razor blades it's probably a bladder or UTI. These are easily treated with antibiotics.
