Think about your question for a bit - it shows from your confidence in your relationship that you have "fingered her" before and like you mentioned, you have had sex prior to this occurrence.
You would have noticed the broken condom in her before, there's no real place it could "hide" (metaphorically speaking).
It sounds to me that you should approach her and mention your discovery and see what she has to say - if she admits it, you will receive a peace of mind. If not, I wouldn't know how to deal with this situation. If I were in your shoes, I would separate with her either way and save her from being "yelled at" or treated like crap... Just merely mention your discover and say "I feel something isn't right about the relationship and I'm moving on"
Of course, there are possibilities, but read the responses from other women here and you will see that a lot of them believe and/or have actually felt something like that happen where they could feel it.

sorry to say, but that definitely would have been noticed by you before. I agree with everything everyone else has said and, not to be too graphic or disgusting, but I would think she would have noticed it when she had her period (as in I mean I think it would have came out then, or if she uses tampons she definitely would have noticed it when she put the tampon in) but I could be wrong about that part.
