I think most cashiers don't really think much. However I am a guy and I was buying some when I was around 20. The cashier was a woman around 40ish and visibly was startled or shaken up by me buying them. I don't know what she was thinking unless I looked 14! Either that or she didn't think 20 year olds had sex. Other than that I don't think they think much of it.
