It's mental and physical. Physically the whole region aches and it can hurt to walk. Mentally, it's like hitting a brick wall for a guy. The come down is rough and the guy will probably be in a bad mood the rest of the night. Some guys, like me, feel betrayed and hurt emotionally by the girl , i dont know how to explain it, but hormonally it causes you to think that way. Believe me, a lot of girls pass off blue balls as guys just being dissapointed or even bitter. But seriously, you gotta be a guy to understand the emotional and physical pain. Id rather be kicked in the balls a half dozen times. Don't ever do this. Only "go there" with a guy if you plan on finishing it. If not, don't "go there" unless you dont like the guy and desire to put that guy in pain. And only if he deserves it.
Well, yes, blue balls does exist, technically.
It's like a charlie horse in your scrotum. Basically a very tight, almost pinching feeling that can be very painful. It can be caused by prolonged arousal/erection, without an orgasm.
The male body knows that arousal generally leads to ejaculation. It's what it's programmed to do, so when a guy is turned on, the body gets everything ready for the eventual release of semen. If that doesn't happen, it can become very painful down there.
However, it does NOT mean that a girl "owes" the guy anything, and she shouldn't be pressured into doing anything. That is the part that was invented by a douchebag. It can be relieved by ejaculation, and 99.9% of men are capable of doing that by themselves.
