That used to happen to me. When she wakes up from a nightmare just hug her and hold her, let her know you're there and that everything's alright. She might try to push you away but keep holding on. Make her feel safe and loved. When you are awake speak to her about the nightmares. If they are constant recurring nightmares she might need to speak to someone to figure out the cause of them and how to stop them.
0 0 0 0I have vivid dreams and the types I have would be considered nightmares. I have NEVER woken up in her state. Is she on any psychotropic medication? Is she particularly stressed about current events in her life at this present time? If this is happening to a point where it is disruptive to sleep quality it may be good to get a sleep study done.

Hold her and rub her back and say calming words
You yourself have to remain calm as well otherwise that will just freak her out more
Since she's scratching herself, I suggest you get a doctor involved
