I don't think his issue in getting hard had much to do with being surprised, if at all. If it was due to that, he would probably quickly get over the surprise and all would be well quite quickly. With my boyfriend, I know that when he's been drinking it takes him longer to orgasm from any sort of sexual contact. Alcohol dulls sensations, so I'd attribute his problem to that versus anything else.

Emotions can play a role in how your junk reacts. I've never had that problem but a couple times, I was nervous when I was with someone the first time and I couldn't finish.
