As you've figured out (I think!), your hymen is the issue. The hymen restricts the vaginal opening, closing it down to a much smaller hole, and exactly how much or how it's constructed will vary from woman to woman. Most typically, during initial penetration, it will be torn in several places, and once torn, will no longer be restricting the entrance. Some women barely have a hymen, or may have a very thin and easy to tear hymen, while for other women, it's thicker and/or covers more of the entrance, and so the latter women normally have some pain and some bleeding during initial intercourse. But once it's over, it's not really a problem ever again.
No, I've never heard of someone being so tight that they hurt the guy's dick. Most men prefer tight women. That being said in a case where she is tighter then he is large the blood that is inflating the erectile tissue is pushed back into the bloodstream when he penetrates you. Therefore, it is not possible to hurt his dick simply by squeezing it, or being too tight. Biting, cutting, or pulling too hard could injure him however.
