No, not if she's on the pill or some form of birth control, unless you don't know her and are afraid she might have an STD.

One girlfriend made me wear a condom the first time we had sex, but never after that.

When I was young and slept around, yes always a condom. Now I don’t use one as I’m in a relationship

If you don't want kids I recommend it.. lots of girls lie about being on birth control to trap a guy with a kid. Plus STD's are on the rise for young people so I would recommend condoms.

No, not for oral or anal or anything that isn’t in the vagina (unless there’s any other forms of contraception being used). As long as she’s comfortable with that though as she may want you to wrap up anyways to prevent stds and things it depends how sexually active you both are.
