It could be either. My understanding though is that usually a fetish is something that is "required" by someone to be turned on. It is probably just something he enjoys, but isn't a "requirement" for him to get sexually aroused. My wife and I enjoy some pee play but I wouldn't consider it a "fetish". I am sure some people would though as it isn't "plain Jane" sex. For us, it is something that adds to our sex life.
