Well I donno... pretty much the same way we do everything else with out penis' "out" if by "out" you mean "there"

The first thing to keep in mind is, unlike most of the penisis you've seen (i'm assuming here) for most of life, your penis isn't erect. That makes everything a thousand times easier in terms of "how do guys do this or that with a penis" than it would be if guys were hard all the time. That's a big part of it. A flacid penis is pretty bendable, smooshable and whatnot, so it just kinda naturally ends up wherever they're room.

But as for sleeping specifically, you may go to bed without an erection, but you're definitely going to have one during the night which will last until after you wake up ("morning wood") So guys do end up having to navigate sleeping with an erect penis. Buuut. no problem..

Key is that an erect penis sticks "up" (towards your chin) more than it "sticks out". the only way an erect penis can really be moved comfortably and naturally is to move it "upwards" so it ends up lying along your stomach, pointed towards your chin. This is the key.

If you sleep on your side or back... it's erect and chilling on or raised slightly above (due to the erection) your stomach.

If you sleep on your side it's basically still in the same position, pointing up erect raised slightly off your stomach.

If you sleep on your stomach than you are indeed sleeping with your erect penis underneath you. In this case, it's in the same position, only it's pressed against both your stomach and the bed underneath you. But since it's pointing "up" then it doesn't "stick-out" it "sticks-up" along your stomach. That prevents it from being uncomfortably squashed...
