A thicker one could hurt initially as the opening would have to stretch. This is where the pain would be felt. The vaginal canal doesn't have that many nerve endings to make childbirth possible. Most of the feeling in the vaginal canal is within the first inch or two, this is why we can wear menstrual products without feeling them.
