A couple of things: despite what some people will tell you, it's real- I can speak to that personally. It's brought on by prolonged periods of arousal and stimulation with no release. The amount of arousal and release required varies, sometimes even for the same person. How often depends on the guys physiology. Essentially it has to do with blood vessels increasing during arousal and if prolonged, it can become painful. I've had it a couple of times and it certainly isn't a nice feeling.

It's not a myth, but it can be prevented.
I don't think I ever got blueballs from first base, but it may be possible. Second base is definitely enough. However, it also must be the case that I haven't came in a few days.

If I go on a date with a girl, and we've never had sex before and I know there's a chance we'll go to first or second base but not beyond, I jerk off earlier that day. This pretty much eliminates the possibility of blueballs.

So it's hard to tell how often it happens. If I have a girlfriend, it usually doesn't happen at all, because the pipes get cleaned every day or two. If I'm single it can only happen out of error if judgement if my part.
However, it should never be used as an excuse to pressure a girl into having sex (especially if you two never had sex before). If a guy tells you that, just tell him he can go to the bathroom to finish off by himself and come back relaxed.
