"not leading someone on" is different from "isn't going to hurt someone"
the first can be avoided by communicating well. On top of that you can be sympathetic and supportive if someone does end up hurt.
the second is impossible to do 100% but to minimize it you need to actually make judgements about how you think someone will react. You would need to turn down consenting partners because you think they don't know what they're doing.
