You have to look at both of them, not just her. In the pic it looks more like anal due to his angle of attack.

It depends where he is in relation to her position. If he is in a fairly typical position for missionary then I assume he's in her ass, since the angle is right for that. It's the preferred position for my girlfriend and me for anal.
