I had had it a few times back in the early make out days. It's 2 things together. 1) During arousal, the testicles are pulled up by muscular tubes called Vas Deferens as they export sperm into the pipeline. They can become strained if arousal lasts a long time. Add to that bouncing as may take place during sexual stuff and there's going to be soreness that lasts for a while. 2) Arousal causes the prostate to engorge with prostatic fluid to prepare for ejaculation. This imparts a congested "full" feeling down there that's not painful but is noticeable. Ejaculation instantly (and pleasurably!) relieves the congested feeling but not the soreness at the testicles which will last for maybe a half hour.

When kids get angry and try to hold their breath, the begin to get blue than purple the longer they hold it. Eventually of course they have to breathe and so they do and they are no longer blue. The phrase blue balls is likely from the same idea. Yeah, it hurts but it's not incapacitating. I'm not sure exactly how it came to be called blue balls. Maybe because venous blood (which is darker than oxygenated blood) becomes pooled in the genital region causing what is known as "vasocongestion," which is what causes the tenderness. If a guy is highly sexually aroused for a long period of time without relief, vasocongestion occurs which leads to the pain.

Yes, and have you ever laid down on your arms during sleep? They start to become bluish and hurt. Yes it hurts. One time I had felt I had been masturbating too much and decided to go cold turkey. After a few days I was in some serious pain. Blue balls are RARE. Most guys who claim they have blue balls actually haven't. They simply were sexually frustrated and called it blue balls.

In order to get blue balls, you would need to get aroused almost to climax and then left without cumming over and over again. But since most guys masturbate there is seldom a case where this happens.

But if you’ve felt some pain down there—or actually noticed your testicles turning a darker shade, and/or swelling. THEN you can claim actual blue balls.

Sexual frustration is the psychological state coming from prolonged periods (months/years) without sexual satisfaction. Blue balls is the result specifically of prolonged *arousal* as I described. Do some googling.
