I think the core issue is she didn't know he was small but I'm guessing he also didn't know it would be a problem. They never communicated their vision of marriage... or at least not fully.

You should have asked for his anatomical information to be disclosed to you before getting married. I have a small penis as well, and I am very upfront about it, for precisely that reason; I don't want to get into a marriage and have my wife be disappointed with me.
