It's definitely true that arousal - especially extended arousal - plays a part most of the time. As I said, when a guy gets aroused, his prostate kicks into high gear, and if it was already pretty full, then that additional production of semen is going to "overfill the tank" and cause pressure and some pain because of that pressure. This increases sexual tension and actually helps a guy build towards and orgasm, and the pain tends to be mostly masked by the endorphins he's getting from the arousal. BUT... if he gets highly aroused, especially for an extended amount of time, such as a long make-out session, and then everything ends without an ejaculation, then the endorphins wear off and the pressure and pain in the prostate remain, and will make the whole area extremely tender. This is what is colloquially referred to as "blue balls" - even though it's the prostate and not the testicles that are in pain, and even though nothing changes color (the term "blue" is an old slang term for "dirty" or "overly sexual"). And getting "blue balls" is why guys generally don't enjoy just making out and stopping - we need the release or we're worse off than before.
