As in why don't they choose to have surgery? Could be that they're only connecting with their gender in a social/emotional way and aren't experiencing as much dysphoria. Could be that they're uncomfortable with the idea of changing their bodies/are scared of the surgery. Could be that they tend towards more gender fluidity and don't actually want a physical change that will be just as dysphoric part of the time. Could be that they plan to have biological children and hormone treatments/surgeries obviously affect that. I'm under the impression that that decision tends to be intensely complex and personal.
