The threat of disease is always real, but the main reason a guy wouldn't use a prostitute is because he doesn't want to be forced to consider the possibility that he lacks the sexual appeal to get it for free. And the problem is that's like 80% of the reason men have sex - just to prove they can get it.
But guys who are in a relationship, guys who are sexual thrill seekers who understand the sexual acts they want aren't things girls would do unless they are being paid, guys on a schedule who just want a quick release without any extra commitment, or guys who acknowledge that paying for it is the only way they'll get it are all likely to use a prostitute.
There can be ample overlap between these, but usually those in the last category are going to try to avoid paying.
