First of all at least for uncircumsized guys the base of our head is *really* sensitive so stroking that area with your tongue can be a great idea.
But don't get tunnel vision on his head.
Play with his whole shaft (using your mouth, not just your hands).

First question is he circumcised or not?

Guys who are circumcised seem less sensitive in that area when it comes to that exact type of pleasure and seem to find it more enjoyable, on average.

Uncircumcised guys seem to be more sensitive when the head is more exposed during an erection as it's usually covered and not as constantly being in direct contact.

I'm myself am in the second category and while I can't say it causes me pain per se... it's a weird kind of pleasure that I could see someone describing as uncomfortable at the least. There's also a chance he's just really sensitive when aroused.

The head being sucked feels great, but what feels even greater is if you go down about another inch as well. The inch or so of the shaft just below the head is the "magic spot" (at least for me) on the penis. So you could try sucking at least that much (head and just below) while he's cumming, and use your hand below that to keep him from going inside your mouth to deep so you don't gag.

Watch the teeth and go slowly at first then build up speed. Stop once in a while to give him a break and your jaw a break 🙂. Ask him if he’s ok, if he wants it faster or slower etc. Remember communication is key.
