It could be both. I honestly think a lot of guys don't know or aren't sure because NOBODY teaches us, including women (often they don't speak up when it comes to sex and what they like/want etc). I think a lot of guys are only going on what they've heard (via porn, online, etc) and really don't know, or have an idea where it is, but have never really seen it and had it confirmed what it looks like. I know myself, I "taught" myself online. I never had a woman show/teach me where it is, what it looks like etc. But also there will be some guys (younger ones especially) that simply don't care to learn either, just depends on the guy and maybe how much he cares about you.
I think a big part of why women don't speak up is there's a lot of shame put on women for wanting a doing sexual acts. I am sure, however, that if the guy asked, they would get an answer. Be genuine. I remember when I was a young teen and we learned about different things, one of the girls didn't know where it was either. I remember when I was young I literally researched about this to know about it and it seems if you really want to know, you can easily find the information. So it seems like it's often because they don't care or if they literally know nothing of it at all - which is sad, especially when it comes to sex ed point of view.
