Most people probably just say something odd like he is just be so glad that he is in bed with his girl in the first place or he do not care what it looks like down there (because most people enjoy sex in the darkness of night or under the cover of blanket) etc but guys obviously have a preference when it comes to girls' private parts. They also think and help her girl for 'designer vagina' etc mainly because of easy availability of porn and pictures of girls who look perfect down there, i. e. no inner labia showing and perfectly waxed and smooth etc. Many of guys - especially inexperienced ones – carefully watch at these images and assume that all girls look like this and be disappointed when his girl is not so perfect. They also ask their girls for surgery to make it neater and trimmer.
To be fair unless they are pretty close and in a relationship the chances of staring right at her pubic zone in full daylight are slim.
That is the thing though, with some things that are pretty obvious like sticking out/big inner labia the guy will almost definitely see/feel it when giving oral (but guy will give a girl oral if they are really very very close).
For me, when my husband look at me – down there in lights on or during the day is a turn on and I feel him very attractive. There is no reason why girls / women worry so much about how they are down there - guys really in general do not mind look, they knows that it is as shallow as they will be happy enough to be down there to love whatever they find. They feel wetness and smoothness and take everything for granted, they are very happy with penetration! They do care about pubic hair maintenance or something. For most guys, the actual look of genitalia is not that important. The only thing most guys really care about is some relative control over pubic hair. Trimmed is enough good because there is no fun in finding an amazing sweet point hidden in the bush – in the darkness of night.

Personally I myself am darker around my pubic area than the rest of the skin on my body, except maybe the inside corners of my elbow on the inside where elbow and arm join.

I only care that the area is natural and not modified with labia surgery, bleached, or pubic hair removed.
