I tend to wear full length coats outside and sit at a desk at work, so the worst that happens usually is I have to stay at my desk a few minutes longer than I would normally. At home I don't care, if my wife sees my boner it just gives her an excuse to have a grope (not that she needs one).
The only time it's really bad is in the swimming pool. It can happen, especially because their tends to be in shape ladies not wearing very much around.
