I assumed it was okay to cum inside a girl because we were not using condoms and now she is mad at me, am I in the wrong?

A girl and I have been seeing each other for a couple of months now and we both hate condoms so we got tested to be safe. Well the first night we didn't use condoms I finished inside of her and she freaked out saying that she was not on Birth control and that I should have asked. I thought that with all that it was safe to assume she was on something since I know you can still get pregnant from stuff like pre-cum.

For everyone worried about a baby, we got some plan B after so it should be fine. We also had a good chat and she apologized for freaking out and said that she had been on BC until just before we started dating but had to get off because of issues.
